Common Causes

In the United states the most common causes of erectile dysfunction:

Heart disease, diabetes, obesity, pelvic surgery, trauma, neurologic conditions, medication related, and priapism (prolonged erection).

How we treat ED

There are a number of different ways to treat Erectile Dysfunction, and each patient deserves a personalized treatment plan based on your degree of ED, your goals, and medical health.

  • A good first step in the treatment of erectile dysfunction is to treat the underlining medical problems that led to ED. This is also called lifestyle changes. Erectile dysfunction lifestyle modifications include: diet, exercise, and weight-loss.

  • Medications that increase blood flow to the penis with arousal is a good first option for the treatment of ED. These can be effective for mild to moderate ED. The class of medications is called phosphodiesterase5 inhibitors (PDE-5i). Examples:

    • Sildenafil (Viagra)

    • Tadalafil (Cialis)

    • Vardenafil (Levitra)

  • Penile Injections - Medications that we teach you how to inject directly into the erectile tissue of the penis for the treatment of erectile dysfunction. These are medicines that increase blood flow to the penis. Examples:

    • Trimix (Papaverine, Phentolamine, Prostaglandin)

    • Bimix (Papaverine, Phentolamine)

    • Prostaglandin (alprostadil)

  • Penile Implants - A surgical solution for patients that do not tolerate more conservative treatments or medications may benefit from penile implant surgery. Penile implants are often called “I-P-Ps” for inflatable penile prosthesis. An IPP is the most commonly implanted device in the U.S. The penile implant is surgically inserted into the penile erectile tissues, and after your incisions are healed, the device is concealed. The main advantage is the erection is reliable, rigid, and requires no medicines.

Penile Implants - Don’t take my word for it!

Penile Implants

Penile implant surgery is a great option for men with erectile dysfunction. Men with erectile dysfunction may not tolerate medications (oral medications like sildenafil or viagra) or penile injection medications (like tri-mix) may not work very well anymore. Some men have side effects from these medicine or they may not like having to take medications prior to sexual intercourse. Penile implant surgery provides men with a reliable, permanent, rigid, realistic, and concealable treatment for erectile dysfunction.


There are two main penile implant companies in The United States. I work with both manufacturers and have experience implanting both company’s products. These links will provide you information on erectile dysfunction and their penile implant products. I look forward to talking to you about either.


Three-Piece Penile Implant

This is the most realistic penile implant - results in the most rigid when inflated and most flacid when deflated.

In the United States, this is the most common penile implant

The three components are:

1- Cylinders, placed inside the erectile tubes of the penis

2- Pump, placed inside the scrotum - gives you total control of inflation and deflation of the cylinders

3 - Reservoir, placed behind the pubic bone - holds the fluid for the cylinders

Two-Piece Penile Implant

This is another option for good rigidity with one less component. All of the fluid is held within the cylinders and pump. The main drawback is the implant tends to be more visible in the flacid state.

The two components are:

  1. Cylinders, placed inside the erectile tubes of the penis

  2. Pump, placed inside the scrotum - gives you control of inflation of the cylinders

One-Piece Penile Implant

This is also called a “malleable” penile implant. It bends down when not in use, and then is repositioned straight for the erect state. This is the most common implant in the world and is the least expensive. Drawbacks include: least rigid and least realistic erection of the three options.

One cylinder is placed on each side of the penis.
